What you'll do

• Ensure VNA orders are correct and medications reconciled
• coordinate home care services with providers
• monitor patient progress and collaborate on care plans
• serve as the contact for VNA matters
• educate patients and providers on home care procedures and eligibility
• assess home care needs
• communicate with VNA nurses and supervisors
• maintain assessments, care plans, progress notes, and patient records
• coordinate hospice care as needed
• follow regulatory requirements and protect confidentiality.
