Primary Care Health Home Registered Nurse Care Manager

The Primary Care Health Home Registered Nurse Care Manager role is onsite in Rolla, MO. Compensation is $26–$37 per hour. The position supports Four Rivers Community Health Center’s Primary Care Health Home patients and population health programs. The nurse coordinates patient care across primary, specialty, and inpatient settings, develops care plans, and uses clinical and population-health information to address care gaps. The role also supports quality improvement and supervises or assists team members working in care management and related programs.

• Build relationships with patients and coordinate with care teams and inpatient facilities

• use EHR and related data systems to coordinate care and evaluate programs
• analyze data to address care gaps
• lead or participate in quality improvement
• safeguard patient information under HIPAA
• assist and supervise program team members.
