What you'll do

• Assess patients and families to determine hospice needs
• develop and revise care plans with palliative nursing actions
• coordinate with the clinical team and supervise involved LPNs and hospice aides
• support patients with pain management
• monitor, record, and report symptoms and changes
• obtain physician orders as needed
• and participate in on-call duties. The schedule is Friday at 5:00 PM through Monday at 8:00 AM.
