The Registered Nurse, Clinical Case Consultant, ECM role is hybrid in California, combining remote work, office work, and field activities. Pay is $45 per hour. The schedule is Monday–Friday, 8:00 a.m.–1:00 p.m., flexible, for 25–29 hours per week. The role supports Pacific Health Group’s Enhanced Care Management program. The nurse provides clinical guidance and oversight to interdisciplinary teams serving high-risk Medi-Cal members with complex medical, behavioral health, and social needs. The role supports coordinated, person-centered care, staff development, quality improvement, and compliance with CalAIM ECM requirements; travel within the hiring county is required for field visits.

What you'll do

• Provide clinical consultation to care teams
• review complex cases and support care planning
• participate in case reviews and conferences
• support ECM implementation and care transitions
• conduct field visits and shadow team members
• coach and mentor staff
• provide direct clinical supervision of LVNs
• collaborate with providers, health plans, hospitals, and community organizations
• review documentation and support compliance and quality assurance
• develop and deliver clinical training
• participate in quality improvement and reporting.
