What you'll do

• Engage individuals and families experiencing homelessness and connect them to the Coordinated Entry System
• complete intake, assessments, referrals, and documentation
• connect participants with housing, healthcare, mental health services, benefits, and other resources
• coordinate with housing and service providers
• lead case conferencing and participate in regional planning
• help prioritize people with the greatest needs for housing
• build landlord and community partnerships
• maintain participant data
• participate in outreach and activities such as the Point-in-Time Count as needed.
