Wraparound Facilitator

The Wraparound Facilitator is an onsite, full-time role in Lafayette, LA, paying $19.25–$20.20 per hour. The position supports youth and families through Volunteers of America South Central Louisiana’s wraparound program. The facilitator coordinates family-driven care by developing and monitoring individualized Plans of Care, connecting families with services and resources, and bringing youth, families, providers, and natural supports together. The work helps ensure coordinated, culturally appropriate support that responds to each youth’s needs and goals.

What you'll do

• Coordinate high-fidelity wraparound services
• develop and update individualized Plans of Care
• coordinate with behavioral health, education, child welfare, juvenile justice, and other systems
• facilitate Child and Family Team meetings at least monthly
• conduct home visits
• monitor services and progress
• maintain timely records and confidentiality.
