Housing Lead Care Manager

The Housing Lead Care Manager role is onsite in San Jose, CA, at Gardner Health Services’ Downtown Resource Center. Pay is $27–$29 per hour. The role supports high-risk patients experiencing homelessness and chronic health, mental health, substance-use, and social barriers as part of the Enhanced Care Management and Community Support program. Working with an interdisciplinary care team, the Housing Lead Care Manager helps members access housing transition navigation and housing tenancy and sustaining services. The role contributes to coordinated support for patients facing housing instability and complex care needs.

What you'll do

• Support high-risk patients experiencing homelessness and chronic disease, serious mental illness, substance use, and social barriers
• collaborate with primary care providers, nurses, care coordinators, social workers, and health educators
• provide support for Housing Transition Navigation Services and Housing Tenancy and Sustaining Services
• apply knowledge of reporting laws, medical terminology, and relevant health and community programs.
