Direct Care Provider

Job overview

The Direct Care Provider works onsite in Peyton, CO. Compensation is $18–$21 per hour. The role supports Sarah’s Home, which serves adolescent survivors of sex trafficking ages 14–17. The provider supervises residents and implements Individual Child Plans and program activities, helping maintain a safe and caring residential environment. Through daily activities, life-skills coaching, mentoring, and collaboration with the residential team, the role supports residents’ program participation and healing process.

What you'll do

• Provide direct supervision of adolescents
• implement trauma-informed care practices and program elements
• facilitate and participate in meals, chores, recreation, homework support, house-cleaning, and life-skills coaching
• support outdoor and equine-therapy-related activities
• monitor behavior and document observations
• support therapeutic staff with behavior plans and safety protocols
• collaborate with case management, therapists, and external partners
• maintain a clean home environment, assist with meals and records, and transport youth to appointments or activities as needed.
