What you'll do

• Coordinate PASRR and PAE processes, documentation, referrals, evaluations, determinations, renewals, and follow-up
• review records for missing or inconsistent information
• advise facility teams on related requirements
• coordinate QAPI data and improvement activities and the Fall Prevention Program
• educate staff
• participate in meetings, audits, surveys, and regulatory reviews
• and coordinate with external behavioral health or neurology consultants as needed.
