Part-Time Registered Nurse Care Coordinator, Home Care

What you'll do

• Serve as a primary contact for clients and families on nursing questions
• conduct initial consultations and 90-day reassessments, including nursing assessments and care plans
• communicate with and supervise CNA/GNA caregivers
• manage care processes, address concerns, and provide coaching and support. During on-duty hours of 7 a.m.–7 p.m., respond to inquiries within 30 minutes
• outside those hours, communication is limited to emergencies. In-person assessments are typically scheduled within 24–48 hours of request.
