What you'll do
- Help clients access medical, dental, vision, housing, financial assistance, and legal advocacy
- support access to disability benefits, Medicaid, and other resources
- distribute medications and support medication compliance
- conduct daily medication checks and document compliance in the EMR
- monitor medication updates and documentation
- complete incidental funds paperwork
- assist counselors with discharge planning.

Why this role
