What you'll do

• Conduct comprehensive assessments and monitor client conditions
• develop and implement nursing care plans
• provide skilled nursing services and treatments
• complete admission, transfer, recertification, resumption-of-care, and discharge paperwork
• coordinate with physicians and other providers
• document clinical notes and visits
• supervise and evaluate paraprofessionals and LPNs
• participate in case conferences and discharge planning.
