The Registered Nurse (RN), Case Manager role is onsite in Hyannis, MA, for a 13-week assignment. Pay is $70–$76 per hour. The schedule is 8:00 AM–4:30 PM, Monday–Friday, 40 hours per week, with rotating days, every other weekend, and rotating holidays. What you'll do
- Screen admissions for discharge planning and care coordination needs
- assess patients using InterQual, Care Maps, clinical pathways, and established standards
- perform utilization review and evaluate admission and continued length-of-stay appropriateness
- develop discharge and care plans with the interdisciplinary team
- coordinate with physicians, patients, families, insurers, and third-party payers
- provide clinical information to payers and identify denial risks
- coordinate appeals, discharge appeal rights, HINN notices, and related documentation
- connect patients with resources for complex psychosocial, financial, and legal needs
- participate in unit rounds and care conferences
- coordinate referrals and transfers
- maintain accurate documentation and support compliance with hospital policies and regulatory standards.

Why this role
Requires a current Massachusetts RN license, at least three years of recent acute care experience, and recent discharge planning and utilization review experience. BSN and case management, CPUM, or specialty certification are preferred.
