What you'll do

• Provide general medical care and care coordination
• conduct histories, physical exams, preventive assessments, and evaluations for care gaps
• address acute and chronic primary care concerns
• document diagnoses and implement care plans
• order or perform appropriate diagnostic testing
• coordinate post-discharge care
• conduct in-person visits in homes, skilled nursing facilities, and public locations, and use alternative visit modalities when appropriate
• participate in pop-up clinics
• provide alternating on-call coverage for urgent lab results and pharmacy inquiries
• document care in the EMR.
