What you'll do

• Perform resident assessments and reassessments
• communicate resident needs to the Health and Wellness Director
• when necessary, participate in hands-on care, medication administration, and nursing tasks
• schedule caregiving and medication technician shifts
• supervise, train, and support care team members
• assist with physician orders
• maintain resident records
• coordinate care planning with onsite home health agencies
• address resident needs and complaints
• coordinate admissions and move-ins
• facilitate communication among residents, families, and healthcare providers.

Why this role

12-hour shift. A valid LPN/LVN license is required; current First Aid and CPR or ability to obtain is required. The posting requires four years of geriatric nursing experience and notes an on-call rotation.
