What you'll do

• Assess patients’ psychological, social, spiritual, educational, developmental, cultural, and discharge-planning needs
• review patient history and physical information
• formulate individualized care and discharge plans
• evaluate progress with the healthcare team
• document assessments, plans, implementation, and evaluation
• advocate for patients.
