Qualified Mental Health Professional / Case Manager

The Qualified Mental Health Professional / Case Manager role is onsite in Houston, TX, with pay of $20–$25 per hour. The case manager works with the clinical team and families to develop strengths-focused service plans, provide skill-building and support, document progress, and coordinate with community partners. What you'll do

• Provide targeted case management and rehabilitative services
• collaborate with the clinical team on service and treatment plans
• deliver skill-building training in homes or the community
• provide face-to-face, phone or virtual support and crisis services
• maintain EHR documentation and timely progress notes
• track outcomes
• engage families
• support daily living skills
• coordinate with courts, probation, agencies and natural supports.
