What you'll do

• Review and implement Individualized Plans and Behavior Support Plans
• provide respectful, person-centered support
• monitor individuals’ physical and emotional health
• follow agency care policies and procedures
• submit required documentation on time
• communicate with supervisors and stakeholders
• attend required training
• follow staffing rotation expectations. Hands-on support may include cleaning, meal preparation, medication assistance, transportation, personal hygiene, and companionship.
