What you'll do

• Assess the physical, functional, psychosocial, and environmental effects of terminal illness
• determine hospice admission and recertification eligibility
• develop, implement, and document care plans
• educate patients and families about disease processes, self-care, end-of-life care, nutrition, and dietary needs
• determine service scope and frequency
• apply infection-control precautions
• provide clinical direction to hospice aides and LPN/LVNs
• document progress
• communicate changes to physicians and other personnel
• monitor cases for payer compliance
• participate in in-service programs and QAPI
• perform on-call responsibilities and services as assigned.
