What you'll do

• Assess home health care needs for referred patients
• liaise with patients, families, physicians, hospital staff, and community providers
• complete intake documentation
• develop individualized Plans of Treatment based on physician orders, patient needs, insurance guidelines, and agency protocols
• coordinate services with care teams
• obtain initial insurance authorizations
• arrange durable medical equipment, supplies, and ancillary services
• educate patients and families about home health services and community resources
• maintain confidentiality and provide culturally sensitive care.
