The Social Worker (Part-Time) role is onsite in Willowbrook, IL, with pay of $25–$27 per hour. The position supports residents at Eden Vista Burr Ridge, part of Vista Senior Living Management’s assisted living and memory care network. The social worker identifies residents’ emotional and psychosocial needs and coordinates resident-centered plans, services, and transitions in collaboration with residents, families, facility staff, and outside resources.

What you'll do

• Provide social services oversight
• coordinate referral-to-admission activities with the Marketing Director
• assist residents and families with admission paperwork and financial-assistance information
• complete PASRR and contribute social history to resident assessments
• develop care plans with residents and families
• lead behavior meetings and monitor medication-related consent and gradual dose reductions
• document progress
• advocate for residents and address concerns
• participate in QAPI and interdisciplinary meetings
• lead discharge planning and coordinate outside resources.

Why this role
