Hospice Chaplain

What you'll do

• Conduct spiritual assessments of patients and family units and document findings. Participate in the interdisciplinary team’s care planning
• share relevant psychosocial, spiritual, physical, and financial observations
• provide individual and family spiritual support at regular intervals consistent with the care plan
• coordinate spiritual care with the hospice team and identified clergy
• maintain accurate records and care plans
• protect confidentiality
• and participate in quality improvement activities as requested.
