Field Registered Nurse — Home Health

What you'll do

• Provide home health visits
• perform comprehensive assessments and document care
• obtain and document physician orders
• initiate and reevaluate plans of care with the Clinical Team Manager
• coordinate with physicians and home-care team members
• complete recertifications, resumptions of care, transfers, and discharges as requested
• assess need for other services
• conduct Home Health Aide supervisory visits.
