Care Coordinator — Oncology

What you'll do

• Conduct structured social-needs assessments and connect patients with community resources
• support care plans and case conferences
• prepare patients for virtual visits and provide basic technology troubleshooting
• schedule follow-up visits and referrals
• prepare charts for provider visits
• track symptom surveys and follow up, escalating clinical concerns as needed
• document interventions and coordination in the electronic health record.
