The professional provides person-centered care, supervision, training, and support, helping individuals work toward personal goals, participate in activities, and remain safe and included in the community.

What you'll do

• Provide individualized support

• lead daily activities and support community participation
• provide personal care as needed
• follow health and behavior plans
• document services
• and respond to medical needs or emergencies.

Why this role

High school diploma or equivalent preferred; requires age 21 or older, one year of relevant direct experience, CPR/First Aid certification, and a valid driver’s license and current auto insurance. Evening and weekend hours may be required as scheduled.
