What you'll do

• Conduct service-needs assessments and ongoing evaluations
• develop, implement, and review individualized service plans
• connect clients and families with community resources
• coordinate care with providers, families, schools, medical professionals, and agencies
• advocate for services and benefits
• monitor progress and adjust services
• assist with discharge and aftercare planning
• maintain clinical documentation and case notes
• participate in team meetings and case reviews
• complete chart audits and support Medicaid compliance
• serve as a liaison
• support clients facing psychiatric, substance-use, or crisis-related challenges.
