The Direct Support Professional III, Coast Supported Living role is onsite in Ojai, CA, part-time, Monday, Tuesday, Wednesday, and Friday from 9:30 a.m. to 1:30 p.m. Pay is $21–$22 per hour. The role supports Momentum WORK’s Supported and Independent Living program for adults with higher support needs. The position implements Individual Service Plans and provides individualized hands-on assistance with daily living, mobility, personal care, household tasks, and community access to promote independence and safety.

What you'll do

• Provide hands-on assistance with activities of daily living, mobility and transfers, personal care, and medication administration according to training and program policy.
• Support Individual Service Plans, meal preparation, household tasks, community access, and community-based activities.
• Maintain service records, report concerns or changes to supervisors, collaborate with families and service providers, and follow emergency procedures and agency policies.
