The Case Manager — Community and Behavioral Services Healing Center role is onsite in Juneau, Alaska. Pay is $30.56–$33.62+ per hour, DOE. The position supports Tlingit & Haida’s Community & Behavioral Services Healing Center and its services for Tribal citizens and other participants. The case manager coordinates participant-centered services, helping people identify needs and work toward goals in their service or treatment plans. The role connects participants with resources, coordinates referrals and providers, monitors progress, and supports continuity across behavioral health, medical, housing, education, employment, and social-service systems.

What you'll do

• Receive referrals and conduct participant intake
• identify needs, barriers, resources, and priorities
• help participants set goals and develop individualized case management plans
• obtain consent and releases
• reassess needs
• coordinate referrals and follow-up
• connect participants with community and Tribal resources
• coordinate communication among participants and providers
• maintain confidential, accurate records
• participate in case consultations and wellness activities.

Why this role

Non-licensed position. Requires an associate degree in a related field or equivalent education and experience, three years of relevant experience, age 21 or older, and a valid driver’s license. Tribal or Alaska Native community experience is preferred.
