What you'll do

• Travel by car throughout the designated service area for appointments and visits
• manage assigned cases, including rush and same-day referrals
• coordinate daily and weekly caseload activities
• conduct onsite case management
• attend physician appointments, hospital and home visits, and case conferences
• discuss diagnosis, treatment, and prognosis with providers
• coordinate communication among case participants
• support recovery, maximum medical improvement, and return-to-work opportunities
• conduct case-specific research and develop action plans.
