What you'll do

• Develop and maintain relationships with hospitals, physicians, case managers, discharge planners, skilled nursing facilities, assisted living facilities, rehabilitation facilities, and other referral sources
• generate Medicare home health referrals and new accounts
• conduct field visits and follow up
• educate healthcare professionals about services
• track referral activity, account development, and sales efforts.
