What you'll do

• Build and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other referral partners
• identify referral opportunities and increase hospice awareness and appropriate admissions
• represent the hospice in the community
• educate partners about services and eligibility
• conduct visits and follow-ups
• manage a marketing schedule and territory plan
• collaborate with clinical and leadership teams
• participate in community events and presentations
• track referral activity and progress
• follow ethical and compliant marketing practices.
