What you'll do

• Provide personal care and assistance with activities of daily living according to the plan of care
• support safe ambulation and transfers
• assist with exercises, snacks, and, in home care, self-administered medications under nurse supervision
• perform directed wound, skin, and bowel care where appropriate
• report changes in patient condition
• document care and observations
• attend care conferences, meetings, and in-services
• participate in the scheduled on-call rotation for home care.
