Full-Time Service Navigator

Job overview

The Full-Time Service Navigator role is onsite in Clovis, NM. Starting pay is $17.37 per hour, and the position is 40 hours per week. Mental Health Resources’ CCBHC programs connect people seeking services with care and community resources aligned with their recovery goals. The navigator reviews intake screeners, identifies needs, and connects clients with appropriate agency programs and community resources. The role also conducts brief interventions when indicated, completes health screenings, documents referrals and follow-ups, and may make 24-hour follow-up calls after acute or emergency department care.

What you'll do

• Meet with clients at intake to review screeners and address social determinants of health
• connect clients with community resources and CCBHC programs
• conduct brief intervention regarding problematic alcohol use when indicated by the AUDIT-C screener
• discuss agency and community recovery programs
• complete health screenings and obtain basic health information
• help navigate primary care referrals
• assess interest in advance directives and connect clients with agency resources
• may complete 24-hour follow-up calls after acute or emergency department care
• document interactions, referrals, and follow-ups in the EHR.
