What you'll do

• Schedule patient visits across clinical disciplines

• assess patient and family needs
• communicate with primary care providers and hospital teams
• obtain discharge instructions and support medication reconciliation
• develop initial plans of care
• obtain insurance authorization
• maintain HCHB tasks, reports, and documentation
• update schedules and reassign visits
• communicate with field staff and physicians
• handle clinical questions within LPN scope
• follow HIPAA and agency procedures.
