What you'll do

• Provide comprehensive acute, chronic, and preventive primary care
• conduct health assessments, physical examinations, and diagnostic evaluations
• develop individualized care plans
• prescribe medications and manage treatment
• order, review, and interpret diagnostic testing
• educate patients and families
• coordinate with clinical professionals, social workers, case managers, and community resources
• document in the EMR/EHR
• participate in quality improvement and population health initiatives
• mentor advanced practice providers and support staff.

Why this role
