The role coordinates with members, families, caregivers, and healthcare professionals and submits service authorization requests to support access to appropriate personal care services.
What you'll do
- Schedule and conduct telephonic and face-to-face assessments
- use standardized tools to determine PCS eligibility
- interview family members and caregivers
- complete assessments within established timeframes
- provide expedited phone assessments as needed
- educate members about selecting PCS providers
- coordinate with prescribers and healthcare professionals
- submit service authorization requests
- protect member privacy and dignity
- participate in appeals and mediation as needed
