Compensation is $143,000–$163,000 per year, with an annual bonus of $40,000. The role supports 21st Century Home Health Services’ home health clinicians and patients across Northern California. Reporting to the Director of Nursing, the supervisor coordinates and oversees patient care delivered by Registered Nurse Case Managers and LVNs. The role provides clinical escalation support, develops staff education, reviews care and clinical records, and coordinates case conferences to promote quality home care and planned patient outcomes.

What you'll do

• Oversee nurses providing care in patients’ homes. Handle incoming calls and follow up to address customer needs. Coordinate nursing training and educational programs. Assess staff learning needs and set goals
• perform administrative call duties and serve as a staff resource. Act as the first point of contact for clinical escalations. Coordinate at least bi-weekly case conferences. Review services, records, diagnoses, medications, procedures, and clinical courses
• guide clinicians and assist with therapeutic goals and plans of care.
