Long-Term Care Pharmacist

What you'll do

• Provide clinical pharmacy services tailored to long-term care residents
• collaborate with physicians, nursing staff, and the care team
• conduct medication reviews and consultations
• dispense medications accurately and provide clinical oversight
• direct pharmacy staff in mixing, packaging, labeling, and prescription order changes
• ensure regulatory compliance
• document and communicate through electronic health record systems
• participate in resident-care improvement meetings.
