What you'll do

• Manage behavioral healthcare for patients and monitor care through treatment modalities
• develop individualized treatment plans during engagement and stabilization
• provide regular consultations in person and occasionally virtually
• collaborate with patient care teams and behavioral health providers
• address medical needs that may affect recovery in relation to social determinants of health.
