Registered Nurse (RN) - Transplant Coordinator

Assesses patients' psychological, physical, environmental, and functional needs. Provides patient and family education specific to the patient’s disease process. Communicates with all members of the transplant team, including hospital staff and referring physician/staff. Participates in data collection/entry in the patient medical record as required by regulatory agencies.
