What you'll do

  • Provide crisis assessment, intervention, education, and support with an assigned team member
  • use approved evidence-based practices and tools
  • collaborate with individuals, families, and treatment providers when authorized
  • link people to basic-needs resources and additional supports
  • arrange follow-up care and appointments
  • assist with commitment petitions and hospitalization admissions
  • connect individuals to 24/7 crisis staff
  • develop safety and crisis plans
  • provide up to 72 hours of intervention, including evening and weekend appointments in homes and communities.
