The Wound Care Nurse (Licensed Practical Nurse) – Weekends role is onsite in Jackson, TN, with compensation of $22–$28 per hour. The position supports West Tennessee Post-Acute, a skilled nursing facility providing transitional care to help residents recover and regain function. The nurse oversees the facility wound-care program, coordinates interdisciplinary care, and provides direct wound care as needed. Assessment, treatment planning, staff education, and monitoring support wound healing, prevention, and patient-centered outcomes.

What you'll do

• Oversee the facility wound-care program and compliance with applicable standards
• conduct skin and wound assessments
• develop, implement, and monitor individualized treatment plans
• provide direct wound care as needed
• collaborate with physicians, nursing, therapy, and dietary teams
• track and document healing progress, infection-control issues, and wound trends
• educate nursing staff
• participate in QAPI activities
• serve as the facility contact for wound-related audits, surveys, and inspections.
