The Clinic RN supports coordinated, evidence-based patient care through nursing visits, patient flow, medication support, referrals, documentation, and patient education. The role contributes to continuity of care and clinic quality initiatives.

What you'll do

• Perform nurse visits
• support clinic workflows and patient flow
• room patients, administer injections, respond to phone and portal requests, and coordinate referrals
• reconcile medications and facilitate refills
• connect patients with community resources
• when assigned, prepare charts, document in the EMR, administer medications and vaccines, coordinate testing, educate patients, and assist with procedures.
