• Build relationships with a panel of approximately 500 Medicare Advantage patients
• provide comprehensive primary care focused on prevention, chronic disease management, and healthy aging
• create personalized care plans

• lead care-plan discussions and transitions
• identify care gaps and support quality measures
• use population health data to prioritize outreach
• document care in Epic or eClinicalWorks
• educate and build trust with patients and caregivers
• contribute ideas to improve workflows and patient experience.
