Utilization Management and Population Health Nurse

What you'll do

• Review utilization cases and apply criteria to initial and continued inpatient services
• identify patients for outreach using risk stratification and disease-management guidelines
• develop individualized care plans with the CPHO team, primary care physicians, and other providers
• coordinate referrals and communication
• monitor progress and barriers to self-management
• provide educational resources
• triage patient calls
• analyze outcomes
• oversee population health quality-improvement studies
• interface with commercial and government entities on care coordination, HEDIS, and quality-improvement activities
• report progress and recommendations to the UM/QM Committee.
