Practice Facilitator

Job overview

The Practice Facilitator role has a hybrid schedule, with two days in the New York City office and three days remote; candidates elsewhere in the United States are also considered. The facilitator partners with community health centers to use data, technical assistance, and continuous quality improvement methods to identify performance gaps and implement changes that improve clinical and preventive care outcomes.

What you'll do

• Design and facilitate virtual and in-person learning sessions
• build collaborative partnerships with health centers
• assess context and capacity
• facilitate data-informed improvement planning
• support real-time data use and Plan-Do-Study-Act cycles
• provide implementation support through team huddles
• document activities and progress in QuickBase
• collaborate with program leads, experts, evaluators, and partners
• communicate emerging issues and promising practices.
