The Field Registered Nurse (RN), Home Care role is an onsite position in Greenfield, MA, paying $40–$42 per hour. Associated Home Care supports seniors and their families with coordinated in-home services. The RN administers each client’s Care Plan and supports health and wellness while maintaining clinical and regulatory standards. The role also guides home care aides through field visits, documents changes in client conditions, and communicates concerns to the care team and families.

What you'll do

• Administer Care Plans in collaboration with the Director of Nursing, clients, caregivers, physicians, and other care team members
• conduct aide orientations, supervision visits, skill checks, and annual performance reviews
• instruct aides during field visits
• review personal care plans
• document services and changes in client conditions or circumstances
• communicate concerns to case managers or client representatives.
