Regional Health Coach Manager

What you'll do

• Supervise Health Coaches and support staffing coverage and consistent workflows across multiple Health Hubs
• provide direct health coaching, health screenings, wellness activities, and chronic disease self-management support
• guide and train staff
• build relationships with healthcare providers, community organizations, and referral partners
• support community events
• maintain accurate Epic EHR records and patient encounter processing in compliance with HIPAA and other requirements.
