What you'll do

• Provide personal care and hygiene according to the patient’s plan of care
• take and record vital signs and weight as ordered
• report and document changes in patient condition
• assist with staff orientation to patients and home settings as requested
• verify schedules and care plans in MobileCare
• submit progress notes, time, travel sheets, and other requested data accurately and on time.
