What you'll do

• Review assessments from referring facilities and identify issues requiring follow-up
• contact referral sources and other key individuals and maintain communication during treatment
• conduct psychosocial assessments and prepare integrated summaries
• develop problem lists and present clinical information to the treatment team
• formulate measurable treatment plans with patients
• participate in multidisciplinary treatment plan reviews
• develop interventions and document clinical records
• lead or co-facilitate group therapy
• complete activity and group plans for the Clinical Director.
